The car has been running perfectly, and fuel mileage has been consistent. Dealership diagnosed a mass airflow sensor fault. They want to replace it for $645. Two months ago the car had the intake system dismantled for carbon-buildup walnut shell blasting. Could a sensor failure be somehow related to the carbon buildup, or its repair two months ago?

You can get the exact same MAF sensor from a Hyundai Sonata 2.5L for your E46. Just do a search for that on here. I'm not sure which model you have but I remember reading it fits the 330i. You can pick it up from Hyundai for a fraction of the price that BMW charges for the same Seimens part number.
These are the several factors that can cause the P0175 code to appear. A clogged or leaking fuel injector. Fuel regulator failure. A clogged or defective mass air-flow sensor. Defective coolant temperature sensor. Thermostat malfunction. ECM requires reprogramming. Unclean or faulty oxygen sensor.
If the MAF doesn't fix it, and there aren't any faults stored for cam sensors or apparent voltage supply issues, I would take a look at the VANOS adjustment unit, especially if you have high miles and/or the symptoms happen mostly when the engine is cold. Good luck!
03/2003 and later E46 use the 6 pin MAF connector. The MS45 ECM gets the Intake Air temperature from the MAF, ergo the additional wiring and different connector, the earlier MS42-43 ECM uses a temp sensor in the intake manifold. Edit - extract page 10-11 Siemens Sensors for Engine Management v 2.0
